This form contains personal and emergency contact details to fill out. The following fields must be filled.
- Personal Details:
- Marital Status: Marital status must be selected from the available drop-down list.
- Select Language: Select the preferred language of the patient.
- Patient Occupation: Enter the occupation of the patient.
- Office Phone: Mobile/Landline of the patient to be entered.
- PAN Card: Pan card number of the patient is to be entered.
- Region: Region of the patient to be entered.
- Emergency Contact Details:
- Emergency Contact Person: The emergency contact person’s name is to be filled in here.
- Relationship: The relationship of the contact person with the patient to be entered.
- Mobile: The emergency contact person’s mobile number is to be filled in.
- Landline: The emergency contact person’s landline number is to be filled in.
- Address: The emergency contact person’s address is to be filled in.
After entering the details mentioned above, the following buttons will be available to continue.
- Save: Upon clicking the “Save” button, the patient’s details entered in the current page will be saved in the Ezovion HMS and redirected to the Ezovion HMS home page.
- Save and Continue: Upon clicking this button, the details of the patients will be saved in the Ezovion HMS and moved to the next tab, “Insurance.”
- Cancel: Upon clicking cancel, and it redirects to the Ezovion HMS home page.
