- Disability ID Number: Enter the disability ID number of the patient.
- UDID Card: Enter the UDID Card number.
- Government Stipend: Enter the amount of government stipend given.
- Place Of Birth: Enter the place of birth of the patient.
- Current Monthly Income: Enter the current monthly income of the patient.
- Type of Residence: Enter the type of residence of the patient.
- Current Job: Enter the current job of the patient.
- Shoulder Width T-Shirt: Enter the shoulder width of the T-Shirt.
- Three-Wheeler usage: Select “Yes” or “No” for the three-wheeler usage of the patient.
- Wheel Chair usage: Select “Yes” or “No” for the wheelchair usage of the patient.
- Save button: Select the Save button to save the above-entered details.
- Reset: Select the Reset button to clear the entered details.
