This screen is to enter the Passport details of the patient in case the patient is a tourist or NRI. This form contains the following fields to be filled up.
- Personal details- Passport Number: Enter the passport number of the patient.
- Visa Issue date- Select the date on which the Visa was issued from the calendar.
- Visa Validity- Select the visa end date from the calendar.
- National ID Number- Enter the national ID number of the patient.
- Citizenship- Enter the citizenship of the patient.
- Port of Entry- Enter the port of entry of the patient.
- Date of Entry- Select the date the patient entered the Country from the calendar.
- Residential Address– Enter the Residential address of the patient.
- Office/Work Address– Enter the office address of the patient.
- Save button- Upon clicking the “Save” button, the entered details will be saved.
